Of course you’ll need Adobe’s Acrobat Reader to see and print out the downloaded two-page nomination form. If you don't have it, you can get it here.

Please note that the nomination period closes on 15 September, so you must ensure the form reaches New Jersey by that date. (Postmarks don't count.) Please note we cannot accept faxed nominations. If you have questions, see the timeline and committee page for information that may be helpful.
