PLEASE NOTE that this PDF form must be printed, filled out, and mailed to Diocesan House; it cannot be submitted as an email attachment. It will not be available as an online form. This is a requirement of the Canons.
Please print the form, fill in the necessary sections, add a signature, and mail to Diocesan House by the deadline of 15 January 2010.
